About Open Records Requests
The City Secretary's Office provides information to the public in accordance with the City Charter and Texas State Law (Public Information Act). The public’s right to know is vital to an accountable, citizen-centered government.
Please select the appropriate link below to submit an Open Records Request online. Initially, you will be asked to create an online account. This it to ensure that your request is submitted immediately and that staff has all the necessary information to process your request and follow up with you in a timely manner. By utilizing your login credentials, you can log into your account to view the status of your request at any time.
For Non-Medical Open Records Requests
If you prefer, you can Download a printable (PDF) Open Records Request. Then fill out and email the downloaded Open Request form to email@example.com or fax it to 972-218-1399. You may also bring the form to City Hall or mail it to the City of Lancaster, City Hall at 211 North Henry Street, Lancaster, Texas 75146.
For Medical Open Records Requests ONLY
Requests from attorneys must be submitted through www.chartswap.com with the EMS Service listed as the Provider.
Non-attorney requests may be submitted in any of the following ways:

Email firstname.lastname@example.org with general information and you will receive a secure email to use for the PHI details
